OBJECTIVE: Evaluate the influence of the BsmI polymorphism of the vitamin D receptor gene on vitamin D levels, and inflammatory and oxidative stress markers in patients with Cystic Fibrosis supplemented with cholecalciferol megadose. METHODS: We performed a single-arm, non-randomized pre- and post-study of 17 patients aged 5 to 20 years with cystic fibrosis diagnosed with vitamin D insufficiency/deficiency 25-hydroxy vitamin< 30 ng/mL. Individuals were genotyped for the BsmI polymorphism of the vitamin D receptor gene and all received cholecalciferol supplementation of 4,000 IU daily for children aged 5 to 10 years and 10,000 IU for children over 10 years of age for 8 weeks. Interviews were conducted with personal data, sun exposure, anthropometric and blood samples of 25-hydroxy vitamin parathormone, serum calcium, ultrasensitive C-reactive protein, alpha 1 acid glycoprotein, total antioxidant capacity, malondialdehyde and kidney and liver function. Inter- and intra-group assessment was assessed by paired t-test Anova test or its non-parametric counterparts. RESULTS: The individuals were mostly male and reported no adverse effects from the use of supplementation, 64 % had 25-hydroxy vitamin levels >30 ng/mL. Patients with BB and Bb genotypes showed increased serum levels of 25-hydroxy vitamin. The group with BB genotype showed a reduction in alpha 1 acid glycoprotein. And individuals with the bb genotype had high levels of malondialdehyde compared to the pre-intervention time. CONCLUSION: It is concluded that variations of the BsmI polymorphism of the vitamin D receptor gene have different responses in vitamin D levels and markers of inflammation and oxidative stress.

AIM: There are few studies comparing the effects of geriatric syndromes and abnormalities in nutritional status and body composition on outcomes among older individuals who have been previously hospitalized. Our study aimed to evaluate the frequency and diagnosis of geriatric syndromes, low muscle quality, and nutritional status in hospitalized older individuals, and to examine their impact on both short- and long-term outcomes. METHODS: This was a prospective study involving older adults (≥60 years). We assessed nutritional status, muscle quality, sarcopenia, and frailty. The outcomes were functional dependence, length of hospital stay, transfer to the Intensive Care Unit, number of readmissions, and mortality. Multivariate analysis was conducted to identify independent risk factors. RESULTS: Even after adjustment for age and sex, increased risk of death was associated with possible undernourishment, sarcopenia, low muscle quality, and frailty (P < 0.05), but not the length of hospital stay (P > 0.05). Our multivariate analysis showed that frailty was independently associated with mortality and functional dependence. Low muscle quality was independently associated with functional dependence. CONCLUSIONS: Geriatric syndromes, abnormalities in body composition, and the overall nutritional status of older patients are important risk factors for adverse outcomes, including functional dependence and mortality. These findings emphasize the need for interventions to improve muscle quality, prevent and treat malnutrition and sarcopenia, and address frailty in hospitalized patients. Studies investigating the association between the phase angle (PA) and frailty are scarce. The PA is considered a practical, low-cost, noninvasive measure for the early identification of this clinical condition. To investigate the association between PA and frailty/pre-frailty, nutritional and clinical aspects in older people. A cross-sectional study was conducted with 51 older people (≥ 60 years). PA was determined using bioelectrical impedance analysis. Frailty profile was determined using the criteria of unintentional weight loss, self-reported fatigue, slow gait speed, low grip strength and insufficient physical activity. Nutritional status was evaluated based on the body mass index and appendicular skeletal muscle mass index (ASMMI). Sarcopenia and sarcopenic obesity were also investigated. Sociodemographic data were collected through interviews. Prevalence of frailty/pre-frailty was 84.3%, with no difference between the sexes. The first tercile of the sample had a PA lower than 6º, which was considered low. No significant difference in PA was found between older people with or without frailty/pre-frailty. In the multiple linear regression analysis, age and ASMMI were shown to be potential independent predictors for PA. Age showed an inverse correlation with PA, while ASMMI showed a direct correlation. No association was found between PA and frailty or pre-frailty.

Introduction: cystic fibrosis is a disease that causes inflammation, oxidative stress and metabolic changes that lead to nutrient deficiency, such as vitamin D deficiency. On the other hand, it is suggested that vitamin D has anti-inflammatory and antioxidant actions. Objective: to evaluate the prevalence of hypovitaminosis D and the association between serum 25 hydroxyvitamin D levels with markers of oxidative stress and inflammation in patients with cystic fibrosis. Method: a cross-sectional study was carried out with 48 patients with cystic fibrosis including children, adolescents and adults in the northeast region of Brazil. Blood collection was performed for analysis of 25-hydroxyvitamin D, calcium, parathyroid hormone, inflammatory process (C-reactive protein (CRP) and alpha-1-acid glycoprotein-A1 (A1GPA)) and oxidative stress (malondialdehyde (MDA) and total antioxidant capacity (CAOT)). The statistical analysis was performed using the “Statistical Package for the Social Sciences”, adopting a significance level of p < 0.05. Results: Vitamin D insufficiency/deficiency was found in 64.6 % of patients. After multiple linear regression analysis, MDA showed an inverse association with blood values of 25-Hydroxyvitamin D (p < 0.05) conditioned by the presence of inflammatory process markers. When only oxidative stress was evaluated, this association disappeared. Conclusion: in conclusion, there was a high prevalence of hypovitaminosis D, with 25(OH)D levels associated with greater oxidative stress when combined with inflammatory markers. Improved vitamin D levels may be an alternative to reduce the damage caused by excess oxidative stress and inflammation in CF patients. (AU)


Introducción: la fibrosis quística es una enfermedad que cursa con inflamación, estrés oxidativo y cambios metabólicos que conducen a deficiencia de nutrientes como la vitamina D. Por otro lado, se sugiere que la vitamina D tiene acción antiinflamatoria y antioxidante. Objetivo: evaluar la prevalencia de hipovitaminosis D y la asociación entre los niveles séricos de 25 hidroxivitamina D con los marcadores de estrés oxidativo e inflamación en pacientes con fibrosis quística. Método: estudio transversal realizado con 48 pacientes con fibrosis quística, niños, adolescentes y adultos, de la región nordeste de Brasil. Se realizó una extracción de sangre para el análisis de 25-hidroxivitamina D, calcio, hormona paratiroidea, proceso inflamatorio (proteína C-reactiva [PCR] y alfa-1-glucoproteína ácida-A1 [A1GPA]) y estrés oxidativo (malondialdehído [MDA] y capacidad antioxidante total [CAOT]). El análisisestadístico se realizó utilizando el “Paquete Estadístico para las Ciencias Sociales”, adoptando un nivel de significancia de p < 0,05. Resultados: se encontró insuficiencia/deficiencia de vitamina D en el 64,6 % de los pacientes. Después de un análisis de regresión lineal múltiple, la MDA mostró una asociación inversa con los valores sanguíneos de 25-hidroxivitamina D (p < 0,05) condicionado a la presencia de marcadores de proceso inflamatorio; cuando solo se evalúa el estrés oxidativo, esta asociación desaparece. Conclusión: en conclusión, hubo una alta prevalencia de hipovitaminosis D, con niveles de 25(OH)D asociados a mayor estrés oxidativo cuando se combina con marcadores inflamatorios. La mejora de los niveles de vitamina D puede ser una alternativa para reducir el daño causado por el exceso de estrés oxidativo y la inflamación en pacientes con FQ. (AU)

ABSTRACT Objective To identify the association between dietary patterns and nutritional status in adolescent freshmen at a public university in Northeastern Brazil. Methods In this cross-sectional study anthropometric variables, body composition and food intake were collected and assessed using the food frequency questionnaire. Dietary patterns were evaluated through factor analysis using the principal component extraction method. Results Two dietary patterns were identified: "Western" pattern, consisting of foods with high energy density and low nutritional value, and the "traditional Brazilian" pattern, with foods from Brazilian cuisine such as rice, beans, corn, roots and tubers, fruits, greens and vegetables. The multiple regression analysis revealed a negative association between the "traditional Brazilian" pattern and both excess weight and body fat in females. Conclusion A healthy dietary pattern with typical local cuisine foods can offer protection to health and should be encouraged.


RESUMO Objetivo Identificar a associação entre padrões alimentares e estado nutricional em adolescentes recém-ingressos em uma universidade pública do Nordeste brasileiro. Métodos Neste estudo transversal foram coletados dados antropométricos, composição corporal e consumo alimentar, verificado pelo questionário de frequência alimentar. Os padrões alimentares foram derivados através da análise fatorial pelo método de extração de componentes principais. Resultados Dois padrões alimentares foram identificados: o padrão "Ocidental", composto por alimentos de elevada densidade energética e de baixo valor nutricional, e o padrão "tradicional brasileiro", contendo alimentos da culinária brasileira como arroz, feijão, milho, raízes e tubérculos, frutas, verduras e legumes. A análise de regressão múltipla mostrou associação negativa entre o excesso de peso e de gordura corporal e o padrão alimentar "tradicional brasileiro" no sexo feminino. Conclusão Um padrão alimentar saudável e com alimentos típicos da culinária local pode conferir proteção à saúde, devendo ser incentivado.

ABSTRACT Objective: There is no consensus as to the best criterion for the evaluation of metabolic syndrome (MS), impairing the estimation of its prevalence. This study aims to compare MS estimates using three recommended definitions for adolescents based on a cross-sectional study nested in the Consortium of Brazilian Birth Cohorts in São Luís, Maranhão. Subjects and methods: A total of 2,515 adolescents aged between 18 and 19 years were evaluated. The criteria of International Diabetes Federation (IDF) and National Cholesterol Education Program Panel III (NCEP-ATP) modified by Cook and cols. (2003) and De Ferranti and cols. (2004) defined SM. To compare the estimates of MS prevalence, the chi-square, Fisher's exact and Cohen's Kappa index tests were used. Results: Among the 2,064 participants evaluated in the final sample. The prevalence of MS ranged from 4.2% (95% CI: 3.3-5.1) to 10.2% (95% CI: 8.8-11.4). When comparing the estimates of MS prevalence in the total sample and by sex, a statistically significant difference was observed. The agreement between the criteria ranged from 0.42 (CI 95%: 0.35-0.49) to 0.55 (CI 95%: 0.48-0.62) in the total sample, 0.33 (CI 95%: 0.24-0.42) to 0.59 (95%CI: 0.47-0.71) among boys and 0.39 (95% CI: 0.26-0.52) to 0.54 (95% CI: 0.44-0.64) among girls. Conclusion: Different criteria provide different estimates for the prevalence of MS in adolescents, reflecting the importance of establishing a consensus.

ABSTRACT Objective: To analyze different anatomical sites in the abdominal region, in order to determine the positional parameter that identifies a higher level of visceral adipose tissue (VAT) and confers a greater cardiometabolic risk. Materials and methods: This is a methodological study in which VAT was evaluated by ultrasonography (USG) in three anatomical sites in the abdomen, while the abdominal circumference (AC) was measured using seven different protocols. Additionally, the glycemic and lipid profile, C-reactive protein, and the presence of systemic arterial hypertension were evaluated. Results: One hundred and six individuals with an average age of 42 (36.8-46.2) years were included. The evaluation of the calibration of the ultrasound procedure for the analysis of VAT by intra- and inter-evaluators showed high reproducibility. The pattern of abdominal fat distribution differed between sexes, with higher mean VAT in males (p < 0.05) and higher mean SAT (subcutaneous adipose tissue) in females (p < 0.005). In the abdominal scan applied to women, higher levels of VAT and lower levels of SAT were observed in the narrower waist region, between the iliac crest and the last rib (p < 0.001). In males, the profile of adipose disposition along the abdomen was uniform (p > 0.05). Correlations between VAT measured by USG and cardiometabolic parameters were relatively stronger in the upper abdomen (p < 0.05). Conclusion: Women accumulate more VAT in the narrower waist region, while men accumulate VAT uniformly across the abdomen. There was relative superiority in predicting cardiometabolic risk in the upper abdomen for both sexes.

Resumo Objetivo Analisar os componentes da Síndrome Metabólica (SM) e a racionalidade na adoção dos critérios utilizados para sua classificação em idosos por meio de uma revisão integrativa Método Trata-se de uma revisão integrativa da literatura nos idiomas português, inglês, espanhol e búlgaro com idosos ( idade ≥ 60 anos), nas bases de dados eletrônicas MEDLINE - via PubMed, Embase, Web of Science e Scopus, sem restrição de tempo de publicação e desenho de estudo, no período de agosto 2022 a janeiro de 2023. Utilizaram-se os descritores controlados do Medical Subject Headings (MeSH) e Descritores em Ciências da Saúde (DEcS) "idoso", "idoso 80 ou mais", "síndrome metabólica", "prevalência", "componente" e "critério". Foram excluídos estudos que não tratavam da temática, publicações não disponíveis na íntegra e que não atenderam à pergunta norteadora. Resultados Após a aplicação dos critérios de elegibilidade, dos 1340 estudos encontrados, foram incluídos quatorze estudos na revisão integrativa. Foram identificados sete critérios de avaliação da SM na população idosa e que a maioria dos estudos utilizou dois dos sete critérios, sendo o critério de IDF citado sete vezes nos quatorze artigos escolhidos e o do NCEP - ATP III mencionado oito vezes. Conclusões Embora as buscas tenham reunido diferentes estudos acerca do assunto, foi possível concluir que os critérios que têm sido propostos para a síndrome metabólica precisam ser revisados e terem seus pontos de corte definidos de acordo com a população estudada.


Abstract Objective To analyze the components of Metabolic Syndrome (MetS) and the rationale for adopting the criteria used for its classification in older individuals through an integrative review. Method an integrative review of the literature in Portuguese, English, Spanish and Bulgarian involving older people (age ≥60 years) on the electronic databases MEDLINE - via PubMed, Embase, Web of Science and Scopus, without restriction on publication date or study design, was conducted from August 2022 to January 2023. The Medical Subject Headings (MeSH) and Health Sciences Descriptors (DEcS) controlled descriptors "elderly", "elderly 80 or older", "metabolic syndrome", "prevalence", "component" and "criterion" were used. Studies that were off-topic, unavailable in full and that failed to answer the guiding question were excluded. Results Application of the eligibility criteria led to the retrieval of 1340 studies, of which 14 were included in the integrative review. Seven sets of criteria were identified for evaluating MetS in the older population were identified and most studies used two of these criteria. The IDF criterion was cited 7 times and the NCEP - ATP III criterion 8 times in the 14 article reviewed. Conclusions Although the searches retrieved different studies on the subject, the results suggest the criteria for MetS should be revised, with cut-off points defined according to the population studied.

ABSTRACT Objective To assess linear growth and weight gain in infants with suspected cow's milk protein allergy with gastrointestinal manifestations, seen at a gastropediatrics clinic. Methods A retrospective cohort study conducted with demographic, clinical, anthropometric and dietary information on 84 infants first seen between 2015 and 2018 and followed-up for six months. Stature-for-age, weight-for-age, and body mass index-for-age in z-scores were evaluated according to the cut off points established by the World Health Organization in 2006. Accelerated growth or catch-up was considered a gain ?0.67 in the z-score of the referred indices, evaluated at 3 and 6 months. Results Median age at baseline was 4.0 months and 88.1% of the infants were already in diet exclusion. Regarding the anthropometric evaluation short stature frequency was 15.5% and the underweight frequency was 8.3% and 3.6% respectively based on the weight-for-age and body mass index-for-age indices. High recovery growth was observed during the follow-up period but was not considered catch up. In boys, the gains in weight-for-age and body mass index-for-age were significant (p=0.02 and p=0.01 respectively) and close to the threshold that characterizes the catch up, 0.58 and 0.59, respectively. In girls, significant gains in stature-for-age and weight-for-age (0.38 and 0.37 respectively, p=0.02 for both) were observed. Conclusion Infants with suspected cow's milk protein allergy with gastrointestinal manifestations should have early access to specialized nutritional counseling to avoid exposure to allergenic food and control allergy symptoms, thereby avoiding malnutrition and ensuring adequate nutritional recovery.


RESUMO Objetivo Avaliar o crescimento linear e o ganho de peso de lactentes com suspeita de alergia à proteína do leite de vaca, com manifestações gastrointestinais, atendidos em um ambulatório de gastropediatria. Métodos Estudo de coorte retrospectivo, com informações demográficas, clínicas, antropométricas e dietéticas de 84 lactentes que iniciaram atendimento entre 2015 e 2018 e foram acompanhados durante seis meses. Foram avaliados os índices estatura/idade, peso/idade e índice de massa corporal/idade em escore-z, segundo os pontos de corte da Organização Mundial de Saúde de 2006. Considerou-se crescimento acelerado (ou catch-up growth) um ganho ?0,67 escore-z nos referidos índices, avaliados em três e seis meses. Resultados No baseline, a mediana de idade foi 4,0 meses e 88,1% dos lactentes já estavam em dieta de exclusão. A frequência de baixa estatura foi de 15,5% e a de baixo peso foi de 8,3% e de 3,6% segundo os índices peso/idade e índice de massa corporal/idade, respectivamente. Houve elevado crescimento de recuperação durante o período de acompanhamento, mas que não configurou catch up. Nos meninos, ganhos no peso/idade e índice de massa corporal/idade foram significantes (p=0,02 e p=0,01) e próximos do limiar que caracteriza catch up: 0,58 e 0,59, respectivamente. Nas meninas, foram observados ganhos significantes na estatura/idade e peso/idade, de 0,38 e 0,37 (p=0,02 para ambos). Conclusão Demonstrou-se que lactentes com suspeita de alergia à proteína do leite de vaca, com manifestações gastrointestinais, deveriam ter acesso precoce ao aconselhamento nutricional para evitar exposição ao alimento alergênico, controlar sintomas e, assim, evitar a desnutrição ou garantir recuperação nutricional adequada.

Abstract As obesity has reached epidemic proportions and given the current recognition of central adiposity as an important cardiometabolic risk factor, several researchers have focused on developing and validating predictive indexes and equations to evaluate Visceral Adipose Tissue (VAT). This study evaluates the applicability of the Visceral Adiposity Index (VAI) for predicting cardiometabolic risk in individuals treated in a hospital In the northeast region of Brazil. The VAT was evaluated by computed tomography (CT) and the VAI was calculated through specific equations for each gender. The sample involved adult and elderly patients of both genders followed up in a cardiology outpatient clinic. The following cardiometabolic parameters were collected: fasting glycemia, glycated hemoglobin, lipid profile, C-reactive protein (CRP) and uric acid. The simple linear regression was used to evaluate the explanatory power of the VAI in relation to the volume of VAT determined by CT. The predictive capacity of VAI in relation to the volume of VAT determined by CT was 25.8% (p=0.004) for males and 19.9% (p<0.001) for females. VAI correlated strongly with the triglyceride (TG) (p<0.001) and TG/high-density lipoprotein (HDL) ratio (p<0.001) and inversely correlated with HDL (p<0.001). Moreover, VAI showed low correlation with the following variables: abdominal circumference, total cholesterol, low density lipoprotein, fasting glycemia, and glycated hemoglobin (p<0.05). VAI was associated with variables considered as cardiometabolic risk factors, but exhibited a low predictive capacity regarding the volume of VAT determined by CT. Thus, caution is recommended in its use in Brazilian individuals.


Resumo Em razão de a obesidade ter alcançado proporções epidêmicas e dado ao atual reconhecimento da adiposidade central como um importante fator de risco cardiometabólico, diversos pesquisadores têm se dedicado em desenvolver e validar índices e equações preditivas para avaliar o Tecido Adiposo Visceral (TAV). Este estudo avaliou a aplicabilidade do Índice de Adiposidade Visceral (IAV) como preditor de risco cardiometabólico em indivíduos atendidos em um hospital no nordeste brasileiro. O TAV foi avaliado por tomografia computadorizada (TC) e o IAV foi calculado através de equações específicas para cada sexo. A amostra envolveu pacientes adultos e idosos de ambos os sexos acompanhados no ambulatório de cardiologia. Os seguintes parâmetros cardiometabólicos foram coletados: glicemia de jejum, hemoglobina glicada, perfil lipídico, proteína C-reativa e ácido úrico. Regressão linear simples foi empregada para avaliar o poder explicativo do IAV em relação ao volume de TAV determinado por TC. A capacidade preditiva do IAV em relação ao volume de TAV determinado pela TC foi de 25,8% (p=0,004) para o sexo masculino e 19,9% (p<0,001) para o sexo feminino. O IAV se correlacionou fortemente com as variáveis TG (r=0,916, p< 0,001) e TG/HDL (r=0,952, p<0,001) e inversamente com o HDL (r=-0,441, p<0,001), além disso, apresentou baixa correlação com as variáveis: circunferência abdominal, colesterol total, lipoproteína de baixa densidade, glicemia de jejum e hemoglobina glicada (p<0,05). O IAV associou-se com variáveis consideradas fatores de risco cardiometabólico, porém exibiu baixa capacidade preditiva em relação ao volume de TAV determinado pela TC, sendo recomendada cautela em sua utilização em indivíduos brasileiros.

RESUMO

A difusão de padrões alimentares ditos "não saudáveis" são fatores de risco independentes para as Doenças Crônicas Não Transmissíveis. Considerando essa importância, foi realizada uma revisão integrativa com o objetivo de analisar as evidências científicas atuais que retratam a qualidade da dieta do idoso no Brasil e seus fatores associados. O levantamento bibliográfico abrangeu publicações brasileiras, no período de 2011 a 2020, nas bases de dados LILACS e Pubmed, sendo selecionados ao final da pesquisa documental 18 documentos originais. Os resultados apontaram que a dieta da população idosa brasileira encontra-se monótona e pobre em nutrientes, sendo um potencial fator de risco para o desenvolvimento e/ou agravamento de doenças. Deve-se, portanto, enfatizar a importância do desen-volvimento de novos estudos, principalmente nas regiões Norte, Nordeste e Centro-oeste, a fim de subsidiar o planejamento de políticas públicas voltadas à alimentação e nutrição desse grupo populacional. Ações concentradas nesse sentido poderão contribuir, sobremaneira, com a promoção da saúde e melhoria da qualidade de vida da população envelhecida.(AU)


The propagation of unhealthy eating patterns is an independent risk factor for Chronic Noncommunicable diseases. Given this importance, an integrative review was conducted, and has as aim to analyze the current scientific evidence that portrays the quality of the elderly's diet in Brazil and its associated factors. The biblio-graphic survey covered Brazilian publications, from 2011 to 2020, in the LILACS and Pubmed databases; 18 original documents were selected after the review. The articles showed that the diet of the elderly Brazilian is monotonous and poor in nutrients, being a poten-tial risk factor for the development and/or worsening of diseases. Therefore, it is necessary to emphasize the importance of developing new es, mainly in the North, Northeast and Midwest regions for the purpose of subsidizing the planning of public policies for food and nutrition for this population group. Targeted actions in this direc-tion may contribute, significantly, with the promotion of health and improvement of the quality of life of elderly.(AU)

ABSTRACT Objective: Cardiovascular diseases represent the main cause of death in chronic kidney disease (CKD). We aimed to evaluate the prevalence and association of the hypertriglyceridemia-waist phenotype (HWP) and visceral adiposity index (VAI) with cardiometabolic risk factors (CR) in patients with CKD on hemodialysis (HD). Materials and methods: The study is based on a cross-sectional design with 265 HD patients in two cities in northeastern Brazil. The VAI was calculated considering the variables body mass index (BMI), waist circumference (WC), triglycerides (TG) and high density lipoprotein cholesterol (HDL-c). HWP was defined as the concomitant elevation of WC and TG. The Poisson Regression Model with robust variance estimation was adjusted considering a hierarchical approach for explanatory variables. Prevalence ratios (PR) were also estimated. The level of significance adopted was 5%. Results: In our study HWP and VAI prevalence's were 29.82% and 58.49%, respectively. In the final model, there was an association between VAI and female gender (PR = 1.46; p < 0.0001) and high body fat (% BF) (PR = 1.33; p < 0.0019). HWP was associated with females (PR = 1.80; p = 0.002), alcohol consumption (PR = 1.58; p = 0.033), obesity (PR = 1.89; p = 0.0001), high %BF (PR = 1.76; p = 0.012) and reduced HDL-c (PR = 1.48; p = 0.035). Conclusion: The HWP stood out as the association with more CR factors, representing a promising method for tracking cardiometabolic risk in HD patients, mainly female.

Resumo Introdução A síndrome metabólica (SM) é o agrupamento de fatores de risco associados à adiposidade central e à resistência à insulina. Objetivo O objetivo desta pesquisa foi analisar a prevalência da SM e os fatores associados em idosos de uma comunidade do município de Recife, Pernambuco. Método Estudo transversal com 166 idosos de ambos os sexos com idade igual ou superior a 60 anos, residentes na Comunidade dos Coelhos, no município de Recife. Para o diagnóstico da SM, foi utilizado o critério do NCEP-ATPIII. A análise dos dados foi efetuada com a utilização do programa estatístico SPSS para Windows, versão 13.1. Na comparação entre proporções, foram utilizados o teste de qui-quadrado de Pearson e o teste exato de Fischer. A razão de prevalência foi utilizada como medida de efeito considerando a SM como variável dependente. Resultados A prevalência de SM encontrada foi de 38,3%. Os fatores associados à SM após ajustes foram: a idade, na faixa etária 60-69 anos (44,4%, p = 0,047), o sexo feminino (43,1%, p = 0,056) e o excesso de peso (57,3%, p < 0,001). Conclusão A SM apresentou uma alta prevalência na população idosa avaliada, sendo associada às características demográficas e ao estado nutricional.


Abstract Background Metabolic syndrome (MS) is a clustering of risk factors associated with central adiposity and insulin resistance. Objective This study aimed to analyze the prevalence of MS and associated factors in older adults living in a subnormal urban cluster located in the city of Recife, state of Pernambuco, Brazil. Method A cross-sectional study was carried out with 166 older adults of both sexes aged ≥60 years residing in the Coelhos Community in Recife. MS was diagnosed through the NCEP/ATP III criteria. Data analysis was performed using the SPSS 13.1 statistical software. The Pearson's chi-squared test and the Fisher's exact test were used to compare the proportions. Prevalence ratio was used as an effect measure considering MS as dependent variable. Results Prevalence of MS was 38.3%. The factors associated with MS after adjustments were being 60-69 years old (44.4%, p=0.047), female (43.1%, p=0.056), and overweight (57.3%, p<0.001). Conclusion High prevalence of MS was observed in the older population evaluated, being associated with demographic characteristics and nutritional status. The group of older adults assessed should be provided with nutritional education and motivation to the practice of physical activities.

OBJECTIVE: The aim of the present study was to evaluate potential predictive factors for the aggravation of COVID-19 in patients hospitalized at a reference hospital in northeastern Brazil. METHODS: A non-paired case-control study was conducted with 235 patients hospitalized at a reference hospital in northeastern Brazil between March and April 2020.The case group was composed of individuals who required hospitalization in the ICU. The control group was composed of patients hospitalized due to COVID-19 who did not meet the criteria established in the institutional protocol for classification as "severe case of the disease" and therefore did not require intensive care. RESULTS: The case group was composed of 84 patients with a median age of 45 years (P25-P75: 36-59); 63.1% weremen; 56.0% were less than 60 years of age; 76.2% had excess weight; 14.3% had a previous heart disease; 46.4% had hypertension; 11.9% had lung disease; and 67.1% took continuous-use medications. The median stay of severe cases in the ICU was positively correlated with weight and BMI only among female patients less than 60 years of age. CONCLUSION: The logistic regression analysis revealed that age older than 60 years and a compromised cardiovascular system were independent predictive factors for the severity of COVID-19

RESUMO

OBJECTIVE: to assess the effects of a problematization educational intervention to promote healthy habits in elderly people with diabetes. METHODS: a randomized clinical trial conducted with 202 individuals drawn for the intervention group and the control group. The intervention consisted of problematization educational activities on a monthly basis for over six months. The control group participated in conventional monitoring at the health unit. Data were collected through semi-structured interviews before and after the intervention, in addition to laboratory tests. RESULTS: after the intervention, when compared to the control group, there was an increase in knowledge about the disease (p<0.001), positive attitude towards self-care (p=0.011), physical activity (p=0.020), diet variety (p=0.002), and lower consumption of oils and fats (p<0.05). CONCLUSION: the intervention performed has a beneficial effect to promote healthy habits.

ABSTRACT Objective: evaluate the relationship between visceral/subcutaneous adipose tissue (VAT/SAT) and glomerular filtration rate (GFR). Methods: A case series study was conducted with 146 male and female adult outpatients at a hospital in Northeast Brazil. VAT and SAT were quantified using computed tomography and GFR was estimated using the formula proposed by the CKD Epidemiology Collaboration. The conceptual model also considered socio-demographic, clinical, anthropometric and lifestyle variables. Results: Females accounted for 71.9% of the sample and mean age was 52.5±13.2 years. Mean body mass index indicated obesity in both sexes (men:30.4±5.9 kg/m2; women: 31.6±6.1 kg/m2). For the same mean age and BMI, men had more VAT and a higher VAT/SAT ratio. Mean GFR was similar between sexes and within the normal range. Simple linear regression analysis revealed that 21.8% of the reduction in GFR in males could be explained by the VAT/SAT ratio (p=0.002). Among females, both VAT alone and the VAT/SAT ratio were predictors of GFR reduction (r2=4.8%, p=0.025 and r2=5.3%, p=0.019, respectively). Conclusion: Mean VAT and VAT/SAT ratio were compatible with abdominal obesity in both sexes and were related to a reduction in GFR.


RESUMEN Objetivo: Evaluar la relación entre el tejido adiposo visceral (TAV) y subcutáneo (TAS) con la tasa de filtración glomerular. Métodos: Estudio tipo serie de casos, en 146 pacientes adultos de ambos sexos, atendidos en un ambulatorio de un hospital de referencia en el Nordeste brasileño. El TAV y el TAS se cuantificaron por tomografía computadorizada y la tasa de filtración glomerular (TFG) estimada por la fórmula del grupo Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI). El modelo conceptual también consideró variables sociodemográficas, clínicas, antropométricas y de estilo de vida. Resultados: Pacientes con edad promedio de 52,5 ± 13,2 años y el 71,9% de sexo femenino. El promedio del IMC en ambos sexos se encuentra en el rango de obesidad (hombres= 30,4 ± 5,9 kg/m2 vs mujeres= 31,6 ± 6,1 kg/m2). Para un mismo promedio de edad e IMC, los hombres presentaron mayor TAV y mayor razón TAV/TAS que las mujeres. El promedio de la TFG fue similar entre los sexos y se encuentra en el rango normal. A través de regresión lineal simple, se evidenció que, en el sexo masculino, la disminución de la TFG puede explicarse en el 21,8% por la razón TAV/TAS (p=0,002). En el sexo femenino, tanto el TAV aislado como la razón TAV/TAS fueron predictores de disminución de la TFG (r2= 4,8%; p=0,025 e r2= 5,3%; p=0,019), respectivamente. Conclusión: Se evidenciaron valores muy elevados de los parámetros antropométricos de obesidad abdominal y promedio de TAV y de la razón TAV/TAS compatible con obesidad visceral en ambos sexos, siendo que estos dos últimos parámetros estuvieron relacionados al descenso de la TFG.
